OFFICE POLICIES
Appointments with Dr. Gaila are scheduled Monday through Thursday. We will supply you with medical history forms to be completed prior to the visit. Payment is required at the time of service Insurance reimbursement is dependent on your policy restrictions. We will assist you in filing the appropriate forms to receive insurance reimbursement. Dr. Gaila is not part of any HMO panel or managed care organization.
